Program Selection Form: Faculty of Education, Music, Science & SBE
The Program Selection form is now available to request a program change for 2015-2016.
This form is to be submitted for the activation of online fall/winter/spring registration. It is also required for the changing of programs (specialization or major) during the academic year at Laurier. Program changes for the following year may be requested using your LORIS account during March 1 to April 28. The Program Selection Form must be used if there has been an absence in registration from Laurier during the winter term.
Students requesting any Honours BBA programs, Honours BSc Health Sciences or Honours Bachelor of Kinesiolgy programs must complete the Request for Program Transfer. An evaluation of your record will be conducted to assess eligibility for these specialized programs.
All current Year 1, 2 or 3 students are automatically confirmed for the Spring term and will be assigned time tickets for February 1. Year 4 students or those returning students that have not attended Laurier during this past fall or winter term must also use this form.
All registered students will automatically be confirmed by Enrolment Services in their current academic program on LORIS, for registration in the next academic year - Fall/Winter.
For most Year 2, 3 and 4 students - no further action is required until the course registration period in June. If you wish to make a change to your program, you may do so by linking to your LORIS account between March 1 and April 28. Your program change will be reviewed by the Office of the Registrar and a decision recorded at the end of the Winter term. If departmental approval is required (for upper year honours programs), please forward email approval from department to Pat Young.
This form is not used for individual course registration (i.e. do not indicate course numbers on this form) - use LORIS once this form is processed.
The program (degree/major) cannot be changed for the term in which it is after the last date to add classes. Program changes will be accepted for an upcoming term.
Minors do not appear on your program registration. Indicate minors on your application to graduate.
