Application to Graduate: Masters, Doctoral, Graduate Diplomas

The printing of your degree for Convocation is done on the basis of the information provided on this form. Should this information be incorrect, you will be asked to pay the cost of re-printing the parchment. A graduation fee will be applied to your account and applications submitted after Jan 15 for spring and Aug 1 for fall, will be charged the graduation fee and a late fee. Apply early. If applying AFTER the deadlines but no later than April 15 for spring and Oct 1 for fall, your late fee should be sent to the Business Office immediately. Grades: All grades, including the thesis grade must be in the Office of the Registrar no later than three weeks prior to convocation.
Please indicate the correct legal form of your name to be printed on the parchment. Use upper and lower case letters, clearly indicate accents, hyphens and proper spacing. If this name(s) or order of names differ from that which is on record in the Office of the Registrar, you will be asked to provide a copy of your birth certificate with this application.

Unable to Attend Convocation:
Degrees/diplomas that are not distributed at convocation, will be held for pick up, by the graduate or their designate (with signed permission letter & identification) in the Registrar's Office for approximately one month after the ceremony. Any remaining degrees/diplomas will be couriered, at university expense, to the last home address on file. Submit the online Non-Attendance Form for degrees/diplomas to be mailed in the week following the ceremony. Students have the responsibility to ensure that their post-convocation contact is correct on LORIS.
We wish to accommodate all of our graduates appropriately in order to ensure full participation in the ceremony. If you have an accessibility need (ie. require a mobility lift, have the support of a service dog, FM system, Braille or alternate format parchment/degree, sign language interpretation, attendant support for stage or seating navigation, etc.) please click here to indicate any needs you may have.
