Application to Graduate: Faculty of Arts - Waterloo Campus

The printing of your degree for convocation is completed on the basis of the information provided on this form. Should this be incorrect you will be asked to pay the cost of re-printing the parchment.

Apply early. If applying AFTER the deadlines, the application and late fees should be sent to the Business Office immediately.
Single Degree/Double Major/Different Faculties:
If you are graduating with two approved simultaneous degrees (not a double major), please submit two applications. If you are receiving a single degree with a double major with two different home faculties (e.g., Psychology (Faculty of Science) and Sociology (Faculty of Arts), please indicate the faculty that you wish to appear on your degree in the comment section at the end of the form.
If you are postponing graduation for any reason you must do so in writing and indicate which future ceremony for which you will meet requirements to graduate.

Letter of Permission Students:
Official transcripts for courses taken on a Letter of Permission must be sent to the address below, and must be received by October 1st for Fall Convocation and May 1st for Spring Convocation.

Unable to Attend Convocation:
Degrees/diplomas that are not distributed at convocation, will be held for pick up, by the graduate or their designate (with signed permission letter & identification) in the Registrar's Office for approximately one month after the ceremony. Any remaining degrees/diplomas will be couriered, at university expense, to the last home address on file. Submit the online Non-Attendance Form for degrees/diplomas to be mailed in the week following the ceremony. Students have the responsibility to ensure that their post-convocation contact is correct on LORIS.
Please indicate the correct legal form of your name to be printed on the parchment.
If this name(s) or order of names differ from that which is on record in the Office of the
Registrar, you will be asked to provide a copy of your birth certificate with this application.
(Use upper and lower case letters, clearly indicate accents, hyphens and proper spacing).
For example: Alex Erin Mc_Dougall-Schone (Umlaut over the 'o' and accent / over the last e)
Calendar Field (below): This field is required and asks that you indicate the published version of the Undergraduate Academic Calendar that you are following to meet requirements in order to graduate. It may be the calendar printed when you started at Laurier or any calendar published since then - up to the current one. Note that ALL program and course regulations from that calendar must be adhered to. If you have followed different sets for two honours disciplines, or wish to advise us of special notes in this regard, indicate this in the Comment Field at the end of the form.
Convocation Participation Needs for Person's with Disabilities:
We wish to accommodate all of our graduates appropriately in order to ensure full participation in the ceremony. If you have an accessibility need (ie. require a mobility lift, have the support of a service dog, FM system, Braille or alternate format parchment/degree, sign language interpretation, attendant support for stage or seating navigation, etc.) please click on either Waterloo Campus or Brantford Campus to indicate any needs you may have. We will do our best to ensure your needs are met and your graduation day is special.
