Academic Probation Acknowledgement Form
This form is to be used in conjuction with a meeting with the Academic Advisor in the Dean of Arts Office to remove the ACADEMIC hold on your LORIS account.
You are currently on Academic Probation because your cumulative GPA is below a 4.00. You shall remain on Academic Probation until you complete at least 4.0 credits after being placed on probation and you have brought your cumulative GPA up to a minimum of a 4.00. Students who do not succeed in raising their GPA to a 4.00 or higher after 4.0 credit attempts will be required to withdraw from the university.
*Please note that OSAP probation and financial holds are very different from Academic probation status. Please contact Student Awards if you have any questions about OSAP probation status or the Business Office for financial holds.
An Academic Hold has been placed on your LORIS account.
In order to access course registration on LORIS, you must submit this form to the Academic Advisors in the Dean of Arts Office. You must also make an appointment to see an Academic Advisor before starting courses in fall term. It is recommended that appointments take place in July or August once course registation is complete. Appointments can be booked online at www.wlu.ca/artsadvising and may either be for in-person meetings or phone appointments. The purpose of the meeting is to ensure that you fully understand your situation and that you are taking the necessary steps to improve your grades.
