Family Changes (New additions to the family, Marriage, Separation/Divorce, Common-Law)



A recent additon to the family, marriage, common law partnership, divorce/separation are all events that can impact your employment benefits.Information is provided below to outline the areas that you may need to consider during one of these life events:

When you wish to change your insurance beneficiary information. Complete the Beneficiary Change Form and return the form to Human Resources to change your insurance beneficiaries.
Pension Plan When you wish to change your pension beneficiary information. Complete the Pension Change Form and return the form to Human Resources.

If removing or changing a spouse, please also complete the Spousal/Declaration Form. If applicable (e.g. your spouse chooses to waive their beneficiary rights), also complete thebeneficiary waiver form. Completed forms can be returned to Human Resources.

NEW ADDITION TO THE FAMILY (e.g. New Baby, Adoption):

Health and Dental Benefits If you are covered by the health and dental plan you are able to add a new your new child to the plan as of your child's date of birth, or adoption. Please ensure they are added to your coverage within 31 days. Complete the appropriate sections of the Benefits Enrollment/Change Form and return this to Human Resources.
Beneficiary Information for Insurance:
  • Basic Life

  • Accidental Death& Dismemberment
Beneficiary Information for Pension.
You may want to add your child as a beneficiary to your current insurance policies.
You may want to apply for additional insurance.
Complete the Beneficiary Change Form and return the formto Human Resources to change your insurance beneficiaries.

Complete the Pension Change Form and returnthe formto Human Resources to update your Pension beneficiaries.

Maternity/Parental/Adoption Leave Contact the Benefits Specialist in Human Resources a few months before your leave. Review your respective collective agreement which will explain eligibility for maternity, parental or adoption leave and supplementary employment benefits.

Contact the Benefits Specialist in Human Resources for an information package.

MARRIAGE

What do I need to do?

Health and Dental Benefits If you are covered by the health and dentalplan you are able to add a new spouse to the benefit plan as of the date of your marriage.

Please ensure your new spouse is added to your coverage within 31 days of the marriage date.
Complete the appropriate sections of the Benefits Enrollment/Change Form and return the form to Human Resources.

As of the date of marriage you may wish to change the beneficiaries for your current insurance policies.

You may want to apply for additional& insurance.

Pension Plan Ontario regulations indicate that a spouse must be named as the primary beneficiary for your pension unless they elect to waive this right.

Beneficiary forms should be completed as of the date of marriage.
Complete Pension Change Form and return the form to Human Resources

Complete Spousal/Declaration Form. If applicable (e.g. your spouse chooses to waive their beneficiary rights), also complete thebeneficiary waiver form. Completed forms are to be returned to Human Resources.
Name Change When you are ready please notify the university if you choose to change your name. To update your Employee File, as well as your health and/or dental benefits, complete and return the Benefits Enrollment/Change Form to Human Resources.

Address Change When you move; as soon as possible. Please update your address through LORIS under the Emloyee Services tab.

COMMON-LAW PARTNERSHIP:

Health and Dental Benefits Eligible employees are able to add a new common law partner to the benefit plan after cohabitating for 12 consecutive months.

Employees are asked to add all new partners within 31 days of meeting the definition of common-law.
To add your common-law partner to your health and dental benefits, please complete the applicable sections of the Benefits Enrollment/Change Form and return to Human Resources.

You may wish to change the beneficiaries for your current insurance policies.

You may want to apply for additional insurance.
Complete the Beneficiary Change Form to change your beneficiaries and return to Human Resources.

Pension Plan After you have been cohabitating for 12 consecutive months your partner meets the definition of spouse under our pension plan.

Complete a Pension Change Form and return to Human Resources.

DIVORCE OR SEPARATION

What do I need to do?

Health and Dental Benefits Upon notification of separation or divorce your spouse's/partners coverage will cease unless the divorce or separation agreement stipulates that coverage must continue.

Please note you can only carry one spouse/partner on the benefit plan.

Employees are asked to notify the University of any change as soon as possible.
Complete the applicable sections of the Benefits Enrollment/Change Form and return to Human Resources.

You may wish to change the beneficiaries for your current insurance policies.
