This form must be printed, completed and submitted to the KPE Main Office (BA 501) PRIOR TO the commencement of any volunteer placement in association with KPE Leadership Activity requirements for graduation. The person/organization supervising a student's volunteer leadership hours must complete the form and the form also requires departmental signature(s) from Kinesiology and Physical Education.
