A student must submit this form prior to the commencement of a proposed activity/experience in order to fulfill the Leadership/Activity requirements for graduation purposes. Form is to be typed.
This form is an interactive electronic version in Word format and can be filled out on screen and then printed. The Undergraduate Advisor of the department will need to sign for approval. Forms should be handed into Tricia in the main office or may be emailed to tlutz@wlu.ca .
