Article 18.5 of the WLUFA Collective Agreement requires each full-time faculty member to submit a copy of an Annual Report of Activities to the Dean by May 5th of each year.
To comply with this article, please complete this form and forward a copy via e-mail attachment to cmahler@wlu.ca by May 5, 2013 with a copy to your Department Chair. Attach additional sheets if necessary. Boxes will expand to fit additional text. Acceptable file formats for attachment are: MS Word or PDF.
Faculty on sabbatical are still required to complete the Report of Activities in addition to their sabbatical report.
