This is the Information and Privacy Commission's guideline document for responses to privacy breaches. A privacy breach may have occurred when records containing personal information are lost, stolen, misdirected, or otherwise accessed by unauthorized persons.
If you suspect that a breach has occured contact the Privacy Co-ordinator (x2037) immediately.
